The stool reaches the anal opening, you can even feel it there. However, no matter how hard you strain, it seems like there is an invisible wall down there not allowing passage.<\/p>\n\n\n\n<p>If this scenario sounds familiar, you might be experiencing a problem known among the public as &#8220;Inability to Relax the Anus,&#8221; and medically as <strong>Anismus<\/strong> (Dyssynergic Defecation). In this article, we will examine this functional disorder, often misdiagnosed as &#8220;chronic constipation&#8221; and treated incorrectly for years, along with the symptoms of Anismus.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What is Anismus? (The Brake and Gas Pedal Paradox)<\/h2>\n\n\n\n<p>For a normal act of defecation, the body must operate two different mechanisms simultaneously and in harmony:<\/p>\n\n\n\n<div class=\"\" data-col=\"2\"><ul class=\"wp-block-list\">\n<li><strong>Propulsive Force:<\/strong> Abdominal muscles contract to push the stool (<strong>Gas Pedal<\/strong>).<\/li>\n\n\n\n<li><strong>Relaxing Force:<\/strong> Anal muscles (Puborectalis and Sphincter) relax to open the door (<strong>Releasing the Handbrake<\/strong>).<\/li>\n<\/ul><\/div>\n\n\n\n<p>In <strong>Anismus<\/strong>, this harmony is disrupted. When the brain gives the command to &#8220;strain,&#8221; the anal muscles reflexively contract instead of relaxing (<strong>Paradoxical Contraction<\/strong>).<\/p>\n\n\n\n<p>That is, the more the patient strains, the more they tighten the anus. It is like pressing the gas and the brake at the same time; abdominal muscles push, but anal muscles lock the path. The result is difficulty defecating and long hours spent on the toilet.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What Causes Anismus? (Incorrect Coding of the Anus)<\/h2>\n\n\n\n<p>Anismus (Dyssynergic Defecation) is a functional coordination error rather than a structural defect. Meaning, there is no tumor or blockage in the anal region; the problem is the disruption of communication between the brain and bowel muscles. Anismus is usually not congenital; it is defined as &#8220;subsequently learned incorrect muscle memory.&#8221;<\/p>\n\n\n\n<p>The 3 most common fundamental mechanisms in the development of this condition are:<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">1. Pain Memory and the &#8220;Fear-Avoidance&#8221; Cycle<\/h3>\n\n\n\n<p>Our body is programmed to avoid pain. Past painful defecation experiences are the most common trigger for anismus.<\/p>\n\n\n\n<div class=\"\" data-col=\"2\"><ul class=\"wp-block-list\">\n<li><strong>Anal Fissure and Hemorrhoids:<\/strong> If the person has previously had an anal fissure or a severe hemorrhoid attack, the brain pairs the act of defecation with &#8220;danger&#8221; and &#8220;pain.&#8221;<\/li>\n\n\n\n<li><strong>Reflex Contraction:<\/strong> Even if the fissure heals, when the person sits on the toilet, the brain develops an unconscious defense mechanism: &#8220;It will hurt, squeeze and protect yourself!&#8221; This causes the pelvic floor muscles (puborectalis) to contract reflexively instead of relaxing.<\/li>\n<\/ul><\/div>\n\n\n\n<h3 class=\"wp-block-heading\">2. Incorrect Toilet Habits and Postponing Defecation<\/h3>\n\n\n\n<p>Wrong habits brought by modern life disrupt the natural rhythm of the intestines.<\/p>\n\n\n\n<div class=\"\" data-col=\"2\"><ul class=\"wp-block-list\">\n<li><strong>Holding Stool:<\/strong> Constantly postponing the need to defecate due to school, work, or hygiene concerns leads to the loss of the sensation of fullness in the rectum (the last part of the intestine).<\/li>\n\n\n\n<li><strong>Excessive Straining Error:<\/strong> People suffering from constipation strain excessively with their abdominal muscles while holding their breath to expel stool. However, if they forget to relax the anal muscles during this time (or accidentally squeeze them), this turns into a permanent &#8220;incorrect straining habit&#8221; over time.<\/li>\n\n\n\n<li><strong>Infancy\/Childhood Period:<\/strong> Strict and oppressive toilet training can also reinforce the &#8220;holding&#8221; habit in children, increasing the risk of anismus in later ages.<\/li>\n<\/ul><\/div>\n\n\n\n<h3 class=\"wp-block-heading\">3. Psychological Factors and Stress (Pelvic Floor Tension)<\/h3>\n\n\n\n<p>Pelvic floor muscles are like the body&#8217;s &#8220;stress barometer.&#8221; Just as we clench our teeth when stressed, some people unknowingly tighten their anal muscles during moments of stress.<\/p>\n\n\n\n<div class=\"\" data-col=\"2\"><ul class=\"wp-block-list\">\n<li><strong>Anxiety and Perfectionism:<\/strong> In individuals with high anxiety levels or overly controlling personality structures, the &#8220;inability to relax&#8221; problem is seen not only mentally but also physically (hypertonia in anal muscles).<\/li>\n\n\n\n<li><strong>Traumas:<\/strong> Past sexual abuse or traumatic physical experiences may cause the person to build an unconscious protective shield in the pelvic region, causing the muscles to remain in a constant state of contraction (spasm).<\/li>\n<\/ul><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Most Common Symptoms of Anismus<\/h2>\n\n\n\n<p>Our patients usually apply to our clinic saying &#8220;I am constipated.&#8221; However, Anismus is not classic constipation (slow bowel function). If you have the following symptoms, you should see a Proctology Specialist:<\/p>\n\n\n\n<div class=\"\" data-col=\"2\"><ul class=\"wp-block-list\">\n<li><strong>Sensation of Obstruction:<\/strong> Stool comes to the anal opening but cannot exit.<\/li>\n\n\n\n<li><strong>Excessive Straining:<\/strong> You use much more force than normal on the toilet.<\/li>\n\n\n\n<li><strong>Incomplete Evacuation (Tenesmus):<\/strong> You feel the need to go again shortly after leaving the toilet.<\/li>\n\n\n\n<li><strong>Manual Intervention (Digital Splinting):<\/strong> This is the most typical symptom. You have to apply pressure to the edge of the anus with your finger to start defecation.<\/li>\n\n\n\n<li><strong>Thin Stool:<\/strong> Because the anus does not open fully, the stool comes out as thin as a &#8220;pencil&#8221; or in pieces.<\/li>\n<\/ul><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">How is Anismus Diagnosed?<\/h2>\n\n\n\n<p>Many patients dismissed with the phrase &#8220;Your problem is psychological&#8221; are actually experiencing a demonstrable physical problem. Standard colonoscopy is not enough for diagnosis; dynamic tests are required:<\/p>\n\n\n\n<div class=\"\" data-col=\"2\"><ul class=\"wp-block-list\">\n<li><strong>Defecography (Defecation Imaging):<\/strong> In this film taken while the patient is straining, it is observed that the anal angle does not correct and the muscle blocks the path.<\/li>\n\n\n\n<li><strong>Anorectal Manometry:<\/strong> The pressure of the anal muscles is measured. It is proven with graphs that the pressure rises (paradoxical contraction) while straining when it should fall.<\/li>\n\n\n\n<li><strong>Balloon Expulsion Test:<\/strong> The time taken to expel a balloon placed inside the anus is measured.<\/li>\n<\/ul><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Anismus Treatment: Biofeedback and Botox<\/h2>\n\n\n\n<p>If there is no underlying anatomical problem (Anal stenosis, prolapse, etc.), Anismus treatment is possible with non-surgical methods.<\/p>\n\n\n\n<div class=\"\" data-col=\"2\"><ul class=\"wp-block-list\">\n<li><strong>Biofeedback Therapy:<\/strong> With computer-aided devices, the patient is visually re-taught which muscle to squeeze and when to relax. &#8220;Muscle memory&#8221; is reset.<\/li>\n\n\n\n<li><strong>Botox Application:<\/strong> In stubborn cases, Botox injection can be administered to relax the muscle.<\/li>\n<\/ul><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">ATTENTION: ARE YOU NOT RECOVERING DESPITE TREATMENT?<\/h2>\n\n\n\n<p>If your complaints do not pass despite doing Biofeedback or dieting; maybe your problem is not Anismus!<\/p>\n\n\n\n<p>Sometimes <strong>Anal Stenosis<\/strong> (Anal Narrowing) or <strong>Hidden Rectal Prolapse<\/strong> can be confused with Anismus. In this case, muscle relaxant treatments do not work.
