As a result, diagnosis may be delayed. This can also delay the detection of perianal or rectal cancer.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What Causes It?<\/h2>\n\n\n\n<p>Irregular bowel habits (chronic constipation, diarrhea), prolonged straining, spending too much time on the toilet, low-fiber diet, insufficient water intake, diarrhea, obesity, and a family history of hemorrhoids are factors that contribute to the formation of the disease.<br>During pregnancy, in addition to mechanical pressure, hormonal changes can cause enlargement and prolapse of the vascular cushions.<br>The condition is more common in individuals who stand or sit for long periods.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What Symptoms Does It Cause?<\/h2>\n\n\n\n<p>Patients typically describe symptoms such as:<\/p>\n\n\n\n<div class=\"\" data-col=\"2\"><ul class=\"wp-block-list\">\n<li>Bleeding<\/li>\n\n\n\n<li>Painful lump<\/li>\n\n\n\n<li>Noticeable swelling in the anal area<\/li>\n\n\n\n<li>Discomfort<\/li>\n\n\n\n<li>Discharge<\/li>\n\n\n\n<li>Difficulty in cleaning<\/li>\n<\/ul><\/div>\n\n\n\n<p>Bleeding is usually fresh, bright red, and painless. It occurs during or after defecation. Some patients complain about soft swellings or \u201chemorrhoid piles\u201d around the anus.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">How Should Diagnosis and Treatment Be Approached?<\/h2>\n\n\n\n<p>Diagnosis is made through physical examination. If no severe pain is present, visual inspection of the anal region and digital rectal examination should be performed.<\/p>\n\n\n\n<p>Patients presenting with rectal bleeding, perianal pain, or a perianal mass should be evaluated for colorectal and anal cancer and inflammatory bowel disease.<br><strong>Flexible sigmoidoscopy or colonoscopy<\/strong> must be used for the differential diagnosis of malignant diseases and inflammatory bowel conditions.<\/p>\n\n\n\n<p>The specialist physician should determine the treatment methods based on the patient\u2019s symptoms, the type of hemorrhoidal disease (internal or external), its grade, and any complications.<br>It should be remembered that the disease often follows a pattern of relief and recurrence during treatment.<\/p>\n\n\n\n<p>Treatment is divided into two main categories:<\/p>\n\n\n\n<div class=\"\" data-col=\"2\"><ol class=\"wp-block-list\">\n<li>Conservative treatments<\/li>\n\n\n\n<li>Interventional treatments<\/li>\n<\/ol><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Conservative Treatment Approaches<\/h2>\n\n\n\n<p>Lifestyle modifications and dietary changes are essential components of treatment.<br>The main principles include:<\/p>\n\n\n\n<div class=\"\" data-col=\"2\"><ul class=\"wp-block-list\">\n<li>Maintaining anal hygiene<\/li>\n\n\n\n<li>Increasing dietary fiber intake<\/li>\n\n\n\n<li>Drinking adequate water<\/li>\n\n\n\n<li>Achieving soft and regular bowel movements<\/li>\n<\/ul><\/div>\n\n\n\n<p>Today, various hemorrhoid creams and medications are commonly used. The specialist\u2019s experience is important when selecting oral medications.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Interventional Treatments<\/h2>\n\n\n\n<p>In second- and third-degree hemorrhoids that do not respond to medication, <strong>non-surgical procedures<\/strong> are used.<br>These include:<\/p>\n\n\n\n<div class=\"\" data-col=\"2\"><ul class=\"wp-block-list\">\n<li><strong>Rubber band ligation<\/strong><\/li>\n\n\n\n<li><strong>Sclerotherapy (injection therapy)<\/strong><\/li>\n<\/ul><\/div>\n\n\n\n<p>Both procedures can be performed under outpatient clinic conditions. They do not require general anesthesia or hospitalization, and patients do not need to take time off work. These procedures are generally painless.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">We prefer sclerotherapy in Our Clinic<\/h2>\n\n\n\n<p>In this method, medication is injected into the base of the hemorrhoid pile.<br>This medication shrinks the vessel by reducing blood flow and causes the hemorrhoidal tissue to regress.<\/p>\n\n\n\n<p>We usually perform <strong>three sessions<\/strong>.<\/p>\n\n\n\n<div class=\"\" data-col=\"2\"><ul class=\"wp-block-list\">\n<li>Symptoms begin to improve within 2 weeks<\/li>\n\n\n\n<li>Complete healing takes approximately 2 months<\/li>\n\n\n\n<li>Itching is typically the slowest symptom to resolve and may last longer than 2 months<\/li>\n<\/ul><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Surgical Treatment<\/h2>\n\n\n\n<p>Surgery is recommended when other methods fail, or in patients with large external hemorrhoids or grade III\u2013IV hemorrhoids.<br>The surgical technique should be selected based on the patient\u2019s clinical condition.<\/p>\n\n\n\n<p>Common surgical methods include:<\/p>\n\n\n\n<div class=\"\" data-col=\"2\"><ul class=\"wp-block-list\">\n<li>Milligan-Morgan<\/li>\n\n\n\n<li>Ferguson<\/li>\n\n\n\n<li>Parks<\/li>\n<\/ul><\/div>\n\n\n\n<p>Laser treatment is also used in some centers.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Why We Do Not Recommend Laser Hemorrhoid Surgery<\/h2>\n\n\n\n<p>We encounter patients in our clinic who previously underwent laser hemorrhoid treatment elsewhere and later developed:<\/p>\n\n\n\n<div class=\"\" data-col=\"2\"><ul class=\"wp-block-list\">\n<li>Anal stenosis<\/li>\n\n\n\n<li>Sphincter injury (gas or stool incontinence)<\/li>\n<\/ul><\/div>\n\n\n\n<p>Revision surgery for these patients is more difficult than the initial hemorrhoid surgery.<br>Therefore, <strong>we do not recommend laser hemorrhoid surgery or the use of energy devices<\/strong> for hemorrhoid procedures.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Our Preferred Method: Ferguson Hemorrhoidectomy (Closed Technique)<\/h2>\n\n\n\n<p>In this method, the excessive hemorrhoidal tissue is removed through an elliptical incision.<br>The wound is then closed with a continuous absorbable suture.<\/p>\n\n\n\n<p>Avoiding inclusion of the sphincter muscle in the suture line is crucial for<\/p>\n\n\n\n<div class=\"\" data-col=\"2\"><ul class=\"wp-block-list\">\n<li>Reduce postoperative pain<\/li>\n\n\n\n<li>Prevent anal stenosis<\/li>\n<\/ul><\/div>\n\n\n\n<p>For patients who have both hemorrhoids and anal fissure, we perform surgical techniques that treat both conditions in the same session.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Hemorrhoidal disease refers to the condition in which the normal hemorrhoidal vascular cushions located in the anal region become enlarged, swollen, and prolapsed.
