So, why doesn’t pilonidal sinus heal?

As a general surgery specialist, I have compiled the most common mistakes that prevent healing and cause pilonidal sinus to become chronic.

1. Mistaking Pilonidal Sinus for a Pimple or Simple Ingrown Hair

One of the most frequent mistakes is assuming that swelling and pain in the tailbone area is just a pimple. However, pilonidal sinus:

  • Forms sinus tracts under the skin
  • Does not heal completely on its own
  • Tends to progress over time

Therefore, accurate diagnosis at an early stage is crucial.

2. Thinking “The Abscess Drained, So It’s Gone”

When a pilonidal abscess drains, pain decreases and the patient feels relieved. However, this does not mean the disease has resolved.

Even if the abscess drains:

  • The sinus tract remains in place
  • Hair and infected tissue stay inside
  • The disease is likely to recur shortly

This misconception leads to delayed proper treatment.

3. Believing Antibiotics Alone Can Cure Pilonidal Sinus

Antibiotics may temporarily suppress active infection, but pilonidal sinus is a mechanical disease, caused by hair penetrating beneath the skin.

Without cleaning or removing the sinus tract:

  • Antibiotics do not provide a permanent solution
  • Infections recur
  • Antibiotic resistance may develop

4. Unnecessary Fear of Surgical Treatment

Many patients delay treatment thinking, “If I have surgery, I won’t be able to move for months.” Today, however:

Harsh shaving with razors:

7. Continuing to Sit for Long Periods

Especially in office workers and drivers, prolonged sitting:

8. Ignoring Excess Weight and a Sedentary Lifestyle

Excess body weight and inactivity:

  • Increase sweating
  • Make hygiene more difficult
  • Deepen skin folds

These factors facilitate both the development and persistence of pilonidal sinus.

9. Seeing a Doctor Only When Severe Pain Occurs

Many patients seek medical help only after an abscess has formed. However, in the early stages:

  • Simpler procedures may be sufficient
  • Recovery time is shorter
  • Recurrence risk is lower

Delayed cases become more complex to manage.

10. Not Following Postoperative Recommendations

Postoperative care is just as important as the surgery itself. Failure to follow medical advice:

  • Delays wound healing
  • Increases infection risk
  • Can lead to recurrence of the disease

The main reason pilonidal sinus does not heal is misjudging the condition and delaying appropriate treatment. With the right approach and early specialist care, pilonidal sinus can be treated successfully.
